Saint Anthony Hospital is an independent, nonprofit, faith-based, acute care, community hospital dedicated to improving the health and wellness of the families on the West Side and Southwest Side of Chicago.
Job Highlights
- This is a grant funded position
- Collaborates with patients, families, and staff on care coordination needs and plans for services/resources as needed
- Gathers pertinent information from medical records, multi-disciplinary team, and family to formulate a comprehensive plan of care
- Documents all interactions and services provided in the client's case file and manages files according to regulatory requirements
Candidate Profile
- Bachelor’s degree in social services or related field preferred
- Bilingual (English/Spanish) required
- Prior experience in care coordination/case management preferred
- Ability to work flexible schedule to address program needs, including both weekend and evening hours
